Hearing Officer Stacy Shulman dismissed an appeal by resident Leslie Malinowski on Dec. 8, 2025, after the appellant failed to appear for a 2 p.m. administrative hearing over a notice of intent to discharge from Corner House Care LLC, located at 1 Griswold Street in Meriden, Connecticut.
Shulman opened the hearing, identified the exhibits and documents to be entered into the record and confirmed that the notice of intent to discharge was dated Nov. 20, 2025. She entered the notice, an acknowledgment of receipt by Malinowski, the appeal letter dated Nov. 24, 2025, a discharge plan dated Nov. 20, 2025, and facility photographs as official exhibits.
Carrie Cardwell, identified in the record as the director of Corner House Care LLC, was placed under oath and testified that she personally delivered the notice of hearing to Malinowski and that he told staff he would not attend. Cardwell said staff spoke with him that day and that "the bus came at 01:45. He got on it and left," indicating he left the premises prior to the hearing.
Shulman noted that the hearing allowed remote participation by cell phone and that tracking evidence showed a delivery attempt of the notice, then stated, "As such, I will be just dismissing this case for his failure to appear." The hearing officer then closed the record.
Amanda Mangiafico, a long-term care ombudsperson for the state of Connecticut, was present during the proceeding; the appellant, Leslie Malinowski, did not appear. The record includes the notice of intent to discharge (Record Exhibit 1), acknowledgment of receipt (Record Exhibit 2), the appeal letter (Record Exhibit 3), the discharge plan (Record Exhibit 4), the notice of hearing (Record Exhibit 5), and Facility Exhibit 7 (room photographs).
The administrative record was closed following the dismissal; no further procedural actions were recorded during the session.
