Her organization focuses on assisting medically complex individuals, particularly seniors and people of color experiencing homelessness, by leveraging local resources and strong partnerships.
Wright emphasized the effectiveness of in-person interactions, which allow her team to build trust and better assess clients' needs. This approach has proven successful, with Eastbay Innovations transitioning 12 clients from skilled nursing facilities to independent housing in just the last three months, and over 300 individuals in the past 15 years. The organization has secured 140 housing units through collaborations with local affordable housing organizations and housing authorities.
To ensure clients' safety and well-being, Eastbay Innovations has developed relationships with local contractors for unit modifications tailored to individual accessibility needs. Additionally, they assist clients in navigating healthcare options and accessing essential services, such as transportation and mental health support. Wright noted that in-person visits often reveal issues that clients may not disclose over the phone, allowing for timely interventions and adjustments to care plans.
